Furniture Donation Form

Please fill out this form to schedule a donation pick up. A representative will contact you to confirm your pick up location, date and time. Please be aware that WE DO NOT accept items that have tears, stains or breaks. They must be in good working order. Additionally, we are unable to take office furniture unless we have a specific need or prior approval given. You will receive a reply from us within 1 business day of submitting your online request. If you have not heard from us please take a moment to check your spam folder and search donate@nulifeoutreach.com, you can also avoid this by adding us to your address book. All of us here at NLO Thank you and we look forward to working with you to help these women and their children get back a life they deserve.

Please list all items you wish to donate. Also note that we ONLY accept items without tears, stains or breaks. We are not able to repair furniture. All items must also be in good working condition.

If the donation items are outside on the ground level the pick up is free. If the items are on your first floor or basement it is $10.00 for pick up. If the items are on upper floors the pick up is $15.00. Once we have offered you a pick up date you will be prompted to submit the floor charge payment. It can be made to our office with a credit card or given to the driver at the time of pickup
